Saint Alphonsus Medical Group (SAMG) and Saint Alphonsus Regional Medical Center (SARMC) have an excellent part time (0.5 FTE) employment opportunity for a Nurse Practitioner or Physician Assistant within a vibrant and growing multispecialty group practice located in Boise, Idaho. 

Saint Alphonsus Medical Group (SAMG) is a physician led and accountable 450+ provider multispecialty group practice that has an excellent opportunity for an advanced provider practitioner to join a newly established Extensivist Clinic . This practice is being established to provide post-discharge transitional healthcare services and priority access for patients with complex medical needs, patients on extensive medication regimens, and patients who have high and rising risk conditions in accordance with applicable scope and standards of practice and within the policies, values, and mission of the organization. This position serves as a transition from acute care to primary care as well as a resource for primary care providers thereby serving as an extension of the Hospitalist Provider and the Primary Care Provider. 

The goal of the Extensivist clinic is to provide excellent patient experience, clinical care, and education by a care team to include an APP and a Pharmacist in an effort to reduce readmissions, ER visits, and unnecessary hospitalizations of the population of patients cared for in the clinic. This clinic supports the overall efforts of care coordination and population health management teams across the healthcare system. 

The qualified candidate will obtain patient histories and perform assessments via observation, interview and examination as well as place orders and interpret lab and diagnostic studies. The APP will be actively involved in the continuity of care starting prior to discharge, continuing through the Extensivist clinic and progressing back to the primary care provider through frequent dialogue and discussion. The APP will provide direct treatment and management of acute and chronic health conditions, including use of prescriptive authority and referral to other healthcare providers or community resources, become comfortable with constant evaluation of the level of care being offered and assessing for overtreatment/under treatment and involvement of other services to get the patient what they need at the appropriate time and in the appropriate setting. The APP will communicate effectively in co-management with the Extensivist Medical Director, Extensivist Pharmacist, and patient's primary care provider. 

The schedule for this opportunity is a traditional clinic work week, Monday through Friday with no less than 36 hours of scheduled patient contact time. There is no call associated with this opportunity.

Requirements for this opportunity include the successful completion of NP or PA Graduate Program (Masters Level), AANP, ANCC or NCCPA certification, a current unrestricted license in Idaho, DEA and a valid controlled substance registration with the Idaho Board of Pharmacy. A strong Internal Medicine or Family Medicine background with two years of acute care experience is strongly preferred. Outpatient primary care experience is also preferred. The qualified candidate must be passionate about adult health issues and demonstrate evidence of comprehensive assessment skills, expert clinical skills and problem solving abilities.

Saint Alphonsus utilizes NextGen Ambulatory EHR and is anticipating a transition to EPIC in the next two to four years, therefore experience with electronic medical records is preferred.
