Springfield, MA: Bilingual Nurse Practitioner invited to join growing nonprofit healthcare team in Western Massachusetts - 20995092

Springfield, Mass area: We invite an experienced NP to join an important healthcare team providing in-home healthcare visits, consultations and treatment. This is a traveling role that will take you around western Massachusetts – a territory that includes Worcester County, and extends south to the CT border, and west to the edge of the Berkshires.

Your important job is to provide occasional, short-term primary care and case management visits with people who are on Medicaid or/or Medicaid -seniors, and persons of varying ages with disabilities, with an emphasis on geriatrics.

Some Spanish language skills are necessary – candidates who are fluent will receive priority attention!

Patient visits are triggered by an acute event, or a significant change in their condition, which necessitates a follow up with a skilled Advanced Practice Provider. As part of a Rapid Response Team, you may encounter urgent care and/or behavioral health issues. Your visit is intended to prevent the need for the patient to have an ER visit or hospitalization. Some visits are post-discharge follow ups. You will take detailed histories, perform physical exams, order appropriate studies and tests, make a definitive diagnosis, develop a treatment plan, and perform any required follow up. Timely, thorough documentation is a key component of this role. You will report to a clinical Medical Director.

You will have a full time weekday schedule (M-F, 8:30-5) and will share a weekend rotation about 8 times/year (when your week will include two weekdays plus Sat & Sun).

We are offering:

  • Secure employment with growing nonprofit organization
  • Interesting mixed practice seeing people in their homes
  • Freedom from office confinement, traveling to see patients
  • Immense job satisfaction providing care where it’s needed most
  • Very competitive salary
  • Convenient weekday schedule
  • Shared weekend coverage, fewer than one/month
  • Paid liability insurance
  • Health, dental, vision and life insurance
  • Paid time off for vacations and your CME
  • Participation in a retirement plan
  • Attractive, vibrant and diverse territory!

Scenic western Massachusetts is a diverse area with industrial cities, historic towns, respected universities, antique shops, winding rivers, and fresh air. This fertile region is marked by lush farms, wineries and beautiful valleys, forests and rolling hills, and friendly residents of varying backgrounds. A prime retirement destination, you will meet interesting people of varying backgrounds, and every generation.

Four beautiful seasons of easily accessible recreational activities invite biking and hiking, fishing, kayaking and rafting, and much more. The fall foliage is spectacular, the apple cider too, and winter sports enthusiasts love the cross-country skiing, and the many trails for snowmobiling and snowshoeing, with downhill skiing nearby too. Enjoy fun summer festivals, a jazzy music scene, and easy proximity to bigger cities like Hartford CT, with an international airport, and Albany NY to the west, plus Boston two hours to the east.
