MercyOne North Iowa Medical Center has an excellent full-time employment opportunity for an experienced Nurse Practitioner or Physician Assistant to join the Population Health Services Organization located in Mason City, Iowa. 

The Population Health Services Organization Post-Acute Advanced Practice Provider (APP) is responsible for the care and oversight of MercyOne attributed patients in the post-acute setting. The provider delivers in-facility and telehealth services that support the identification and care coordination of attributed patients in selected partnered post-acute care facilities. The provider will also be available to provide consultation within the ED and/or acute care setting regarding care transition planning to ensure the most clinically appropriate, least restrictive next site of care for attributed patients. 

The objectives of this role are to decrease unnecessary post-acute utilization and length of stay for patients in value-based contracts as well as work to reduce ED and acute care readmissions from the post-acute care setting. This is a full time (1.0 FTE) position, Monday through Friday, 8:00 am – 5:00 pm. 

Support staff includes other experienced physicians and APPs, nurses, a post-acute health coach, and two dedicated CMA/LPNs to support the practice, as well as a healthcare community across MercyOne. These APPs will help lead a multi-disciplinary team caring for their patients in a skilled level of care, ensuring that their patients successfully transition back home. 

Ideal candidates are innovative, hard-working, and excited about developing this new program and improving the quality care for the population we serve. Success within this role will be illustrated through the program’s development as well as improved quality, patient experience, and utilization outcomes. 

The ideal candidate will have experience functioning as an APP; however, we are open to new graduates with specific experience in skilled care or long-term/nursing home care. Experience with the geriatric population is preferred. 

Minimum qualifications include graduation from an AMA approved Nurse Practitioner program or graduate of an accredited Physician Assistant training program; current Iowa Nurse Practitioner or Physician Assistant license; member of National Practitioner Data Bank; meet credentialing process of hospital where practicing; possess federal and state Controlled Substance prescribing authority.
