Physician Assistant – Geriatric Medicine (Per Diem) Opportunity - 3235 (23642183)

St. Peter's Health Partners is seeking an experienced Per Diem Nurse Practitioner or Physician Assistant for its established PACE program located in Latham, New York. 

Trinity Health is the largest provider of PACE (Program of All-Inclusive Care for the Elderly) services in the country with 13 PACE organizations operating in 9 states. PACE is designed to provide the entire continuum of care and services to frail seniors, helping them to maintain their independence in their home and community for as long as possible. 

This opportunity is for coverage of the Latham PACE Center; coverage is needed for one to two days per week. 

Interested candidates should have the following: 

  • Board Certification
  • Three years of NP/PA experience with an elderly population is required
  • NYS license and DEA required
